A. If the Qualifying Certificate is issued 30 days or more prior to the date of birth of a child of a member and his wife, and the child is born alive but dies before attaining the age of 61 days, a $1,500 Family Fraternal Benefit will be paid to the member. The claim for this $1,500 benefit must be received at the Supreme Office of the Knights of Columbus within three years of the date of the child’s death.
B. If an unborn child of a member and his wife dies 20 weeks or more after conception, and the Qualifying Certificate is issued 30 days or more prior to the date of miscarriage or the date of stillbirth, a $750 Family Fraternal Benefit shall be paid to the member. The claim for such benefit must be supported by medical evidence. No benefit will be paid if there is evidence that induced abortion terminated the pregnancy. The claim for this $750 benefit must be received at the Supreme Office of the Knights of Columbus within three years of the miscarriage or still birth. The Order affirms that life begins at conception. In this instance, the benefit requires a certified death certificate for the stillborn child, hence the 20-week rule.
C. If the Qualifying Certificate is issued 30 days or more prior to the date of birth of a child of a member and his wife, then, regardless of the physical condition of the child, a $5,000 20-Year Payment Life plan will be issued on the child’s life, provided that each of the newborn child’s living brothers and sisters, who are under 18 and who have not been declined for Knights of Columbus insurance coverage, is the primary insured under a premium paying or fully paid up certificate issued by the Order. The member’s application for this $5,000 20-Year Payment Life plan must be postmarked before the newborn child reaches the age of 61 days. Such application must include the initial premium payment. When an application on a child under 61 days is completed and you believe that the child is uninsurable according to the usual underwriting practices of the Order, and the applicant wishes to request insurance under the Family Fraternal Benefit program, you should write the following under Remarks: "Application submitted under the Family Fraternal Benefit program. Insurance is not effective until the insured is 61 days old" The applicant must initial this statement. This certificate, which will be issued without evidence of insurability at the Order’s standard premium rate, will take effect when the newborn child is 61 days old.
D. If a natural child of a member and his wife, or a member’s adopted child, or a member’s stepchild, who is older than 60 days and younger that age 18, is uninsurable according to the usual underwriting practices of the Knights of Columbus, the Order will, upon receipt of the member’s application and initial premium, issue a $5,000 20-Year Payment Life plan on the child’s life. This certificate will be issued without evidence of insurability at the Order’s Table P substandard premium rate.
To be eligible for this 20-Year Payment Life certificate, the following requirements must be met:
1. The Qualifying Certificate must have been issued 60 days or more prior to the application for the certificate.
2. Each of the child’s living brothers and sisters, who are under 18 and who have not been declined for insurance, must on the date of application be covered as the primary insured under a premium paying or fully paid up Knights of Columbus certificate for the past 60 days or more.
3. At least 90 days, but not more than 120 days, must have passed since the completion of the original declined application for insurance on the child’s life. (This requirement can be waived with Medical Department approval).
4. The child who is insured under Benefit C is not eligible for this benefit.
E. If the natural child of a member and his wife, or a member’s adopted child, or a member’s stepchild, who is between the ages of 3 and 18 years, is mentally disabled but otherwise in good health, the Order will, upon receipt of the member’s application and initial premium payment, issue a 20-Year Payment Life plan for an amount up to $5,000 on the child’s life at its standard premium rate. (This benefit replaces Resolution 149.)To be eligible for this 20-Year Payment Life plan, the following requirements must be met:
1. Each of the living brothers and sisters of mentally disabled child who are under 18 and who have not been declined for insurance, must be covered as the primary insured under a premium paying or paid up Knights of Columbus Certificate.
2. The Qualifying Certificate must have been issued 60 days or more prior to the application for the certificate.
3. The total amount of Knights of Columbus insurance in force on the mentally disabled child’s life, including the coverage provided under this 20-Year Payment Life plan, cannot exceed $5,000.
If a child who is covered under Family Fraternal Benefit D meets all of the requirements for eligibility for Family Fraternal Benefit E, upon application, future premiums for such coverage will be reduced from Table P to standard.
These Family Fraternal Benefits are made available by the Order effective August 1, 1994. Benefits A and B are provided at no cost, and Benefits C, D, and E are offered on a contractual premium-paying basis. The Family Fraternal Benefits may be amended or discontinued at any time by action of the Order’s Board of Directors.
